New versions of immigration medical forms

The Customs & Immigration Service would like to inform permit applicants and employers looking to employ permit applicants that new versions of the immigration medical forms are now available on the Customs & Immigration website (under the ‘Immigration Forms’ tab).

Please use these forms in applications going forwards and submit to the King Edward Memorial Hospital in the usual manner.

The Customs & Immigration website should be consulted for latest versions of all immigration forms.
